Pillows are a constant source of frustration for many. In addition to aggravating the neck, lots of pillows can also affect the shoulder. The shoulder should not be placed at 90 degrees to the mattress, because it can cause spasms in the trapezius muscles.Trapezius muscles perform a variety of actions, including acting as a platform to raise the skull, which is very important to relax during sleep completely. If the shoulder is positioned slightly forward on the mattress in front of the bed, assuming that the mattress can hold this position, the risk of putting pressure on the trapezius and forcing the scapula (shoulder blade) to the column is reduced. By contrast, when in the 90 degree position, sleepers can wake up with neck pain and upper arm and altered sensitivity to little and ring fingers.

You should also consider your size and body frame when buying the best neck pillow for yourself. If you are smaller or taller than average a standard neck pillow may not be right for you. The reason is that your neck may not fit the pillow adequately causing you to either not get the best support or even forcing your neck into incorrect posture. If you are too small for a standard size pillow or have a very short neck the cervical roll may push your head forward. This simply causes head flexion and may straighten your curvature when sleeping on your back versus putting the smooth curvature into the neck.
